About The Position

The primary function of the Social Worker is to assess psychosocial needs of the patients and families and identify appropriate interventions to support those needs. The Social Worker provides also may provide crisis intervention and supportive counseling to patients and families in all areas of the hospital. The Social Worker is an advocate for the patient and family and provides counseling and support to patients and families dealing with chronic illness and new-onset diagnoses. The Social Worker additionally provides care coordination services to the patient and family and will refer them to services in the hospital for support, as well as appropriate community-based organizations. The Social Worker is a member of the multidisciplinary team and collaborates with the patient's medical providers to support the patient and family. Requirements

  • Completes and maintains all competencies and trainings as required for role.
  • Master's Degree from a school of Social work accredited by the CSWE
  • Relevant Experience
