The Behavioral Health Social Worker assesses and plans interventions to support patients coping with social, emotional, economic, and environmental challenges across the continuum of care. This role conducts psychosocial assessments with patients and families, coordinates internal and external services, participates in interdisciplinary case reviews, and collaborates with the healthcare team to support treatment planning and progress. The Social Worker provides individual, family, and group therapy, assists with discharge planning by identifying appropriate community resources, and facilitates referrals related to housing, transportation, healthcare, and social or spiritual needs. This position also supports staff development through training and supervision while exercising sound clinical judgment, working independently, and responding effectively in a fast‑paced behavioral health environment.
