Position Summary: Coordinates admissions/discharges and organizes social service programs to meet the needs of clients.

Requirements

B.A. in Social Work or Comparable Bachelor’s degree, as well as a State Certification
One year of supervised Social Work experience in a health care setting working directly with individuals and/or in a community outreach type role preferred
Excellent verbal and written communication
Reasoning Ability - Ability to apply common sense understanding to carry out detailed written or oral instructions
Customer and Personal Service - Knowledge of principles for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
Service Orientation - Actively looking for ways to help people
Social Perceptiveness - Being aware of others' reactions and understanding why they react as they do
Active Listening - Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times
Must follow all Policies and Procedures of St. Ann Center for Intergenerational Care

Responsibilities

Introduce clients and/or caregivers to the program and provide necessary enrollment information including taking them on a guided tour of the facility.
Keep an updated list of the client staffing which are to occur one month after the client begins and every 6 months after that, as well as, attend and help to facilitate such meetings.
Review and implement intake policies and procedures.
Stay up to date on current county and state policies regarding the populations in which you will be working.
Stay in constant contact with both the nurse and unit directors and other staff in the building regarding client related referrals or issues.
Appropriately refer clients and caregivers to other community agencies, which may be of service to their specific situation.
You may or may not choose to supervise students from different area colleges during their work studies here.
Document intervention, 1:1 sessions, issues etc. as well as, all known or suspected cases of abuse and take appropriate action.
Assist with any transfer or discharge of clients as needed.
Communicate with other agencies involved in the client’s lives such as social workers, family, and other community agencies, included at times, setting up their transportation to the center.
Assist in the units during times when supervision may be low.
Check regularly with the clients on their attitude toward the program and see if there is anything, more which can be done to improve satisfaction.
Attend conferences and act as a representative of St. Ann’s informing the public of our mission and the services we offer.
And other duties as may be assigned by the supervisor.
