About The Position

Description Standard Functions/Duties: The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Provide follow-up and/or short-term case management, as needed, to determine outcomes and provide assistance in locating resources and services. Develops action plans that are short term, to organize and direct in-home supportive services for individuals with a limited support network according to ADRC scope of work. Represents the ADRC of Fond du Lac County to the community through professional interaction, public speaking, presentations, and community groups Provide information and assistance, options counseling, enrollment/disenrollment counseling, long-term care program eligibility determination within ADRC scope of work Provide older adults and adults with physical and/or developmental disabilities information about a wide range of community resources. Inform and educate people about their long-term care options; assist in connecting them to programs, services and public benefits. Other Functions and Duties: Administers Long-Term Care Functional Screen to determine an individual’s functional eligibility for managed long-term care and IRIS. Be available during the assigned hours for in office or community work. Knowledge, Skills, and Abilities: Knowledge of interviewing, communication, and counseling. Knowledge of Federal and State laws, rules, standards and regulations pertaining to practice area. Maintain confidentiality, exercise sound judgment in confrontational and stressful situations, and follow instructions, to complete tasks and to work closely with the supervisor and others

Requirements

  • A Bachelor's or Master’s degree in Social Work, Human Services, or a closely related Human Services field.
  • Approval from WI DHS if the education and/or experience requirements are not met.
  • Wisconsin’s Long Term Care Functional Screen (LTCFS) certification is required within 3 months of employment.
  • In evaluating candidates for this position, the County may consider a combination of education, training and experience, which provides the necessary knowledge, skills and abilities to perform the duties of the position.

Nice To Haves

  • Candidates with a Social Work Certification are strongly preferred.
  • Non-social work candidates with a qualifying degree will be strongly encourage to pursue the necessary education to gain Social Work Certification.
