About The Position

The Social Worker 2 role within the University Hospitals Seidman Cancer Center offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues. We currently offer an opportunity for a full time, Social Worker to support adult bone marrow transplant patients. Serves as a resource within the social work department and the hospital.

Requirements

  • Master's Degree from an accredited school of Social Work (Required)
  • Licensed Social Worker (LSW) (Required)
  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)
  • Works autonomously (Required proficiency)
  • Communicates effectively with persons of various backgrounds (Required proficiency)

Nice To Haves

  • Hospital/Health Agency experience.
  • Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility)
  • This Social Worker will support bone marrow transplant teams within Seidman Cancer Center. Responsibilities will include performing psychosocial assessments for patients considering transplants, supporting patients with emotional and mental health issues, providing various social services resources, etc.

Responsibilities

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
