About The Position

The Medical Mgmt Social Worker 1 provides patient and family assessments in order to design effective treatment programs and follows the patient and family through the continuum of services. The Medical Mgmt Social Worker 1 provides supportive counseling to patients and families, assesses patient discharge needs and coordinates discharge services. Relies on education, experience, professional training and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision.

Responsibilities

  • Screening of patients/clients for services and intervention
  • High risk screening indicators are used to appropriately to identify population at risk
  • Risk/screening assessment is concluded according to required format and within required time frames
  • Facilitate a smooth and timely discharge o patients from the acute care setting
  • Keeps up to date with all laws, regulations and policies related to social work
  • Refer all physician and complex issues to the director and/or manager in a timely manner
  • Clinical record contains documentation of appropriate interventions, and plans for ongoing services
  • Provide assistance to patient/family with adjustment to illness and medical environment
  • Actively participates in IDT regarding discharge needs/plans
  • Community resources are effectively worked with/coordinated and keeps current on new resources in the community
  • Interactive communication in planning and implementing of discharge process is conducted
  • Interacts in a positive, competent and respectful manner with patients, families, caregivers and staff, co-workers, physicians and community agencies
  • As an expert resource for co-workers on complex cases, collaborates with other social workers to think outside of the box
  • Appropriate referrals are made for interventions for patient/family
  • Community resources are used appropriately
  • Documentation contains all referrals and clinical interventions related to alcohol/substance abuse, neglect, abuse, domestic violence within clinical record within one working day
  • Demonstrates the patient advocacy role through communication and documentation
  • Effectively manages assigned caseload, utilizing strong organizational skills and working independently
  • Documentation of education and evidence of team work is included in patient record by established time frames
  • Resources with needed Information are found and understanding of the impact of clinical data occurs
  • Demonstrates same by documentation of family/patient understanding and response to clinical information
  • Documentation included related to patient and/or family understanding of barriers related to appropriate care and transition planning
  • Other duties as assigned are completed
