About The Position

Pay will be based upon experience, certifications, skills and education. The overall purpose of this job is to advocate for residents and to assist in meeting mental, social, rehabilitation, financial and discharge planning needs of residents and families. The primary responsibilities include organizing, implementing, evaluating, and directing social service programs in accordance with current existing federal, state, and local standards to ensure medically related emotional and social needs of the residents are met. Requirements

  • Bachelor’s Degree from an accredited school of social work required.
  • Two (2) years of experience working on a multidisciplinary team in a hospital, nursing home or community-based setting.
