About The Position

This Social Work Intake Coordinator position provides patient care to the diverse outpatient population of the Child and Adolescent Outpatient Psychiatry clinic. Duties will include providing comprehensive care through assessment, psychotherapy with individuals, families and groups and referral to outside services as needed. Responsible for all aspects of therapy and psychological assessment, including case formulation, diagnosis, case management and presentation and report writing. Develops with the patient/family an individualized plan of care that addresses age specific, physical, psychosocial, cultural and educational needs and modifies the plan as patient status changes. Responsible for conducting psychological screenings with potential patients and their families. Documents and communicates expected outcomes with all members of the Interdisciplinary Team, patient and family to foster identification of patient centered, measurable and time estimated goals. Coordinates care with PCP and various outside agencies and obtains referrals into additional programs to support the patient's recovery. The Social Work Intake Coordinator will work with admin staff on providing clinical information for patient to be admitted to our program.

Requirements

  • L.M.S.W., L.C.S.W. or eligible
  • 3 years relevant post-master's experience required.
  • New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit.
  • Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
