One California Job Summary and Responsibilities Position Summary: Under direct supervision the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the Supervisor Care Manager
Requirements
- Two years of experience providing social service in one or more of the fields of aging health and long term care services required
- Bachelors degree in social work from an accredited school required (BSW)
- Current CA Drivers license
- BLS Certification from an American Heart Association program required
- Ability to relate to clients and families
- Excellent communication and presentation skills
