About The Position

The Social Work and Workforce Support Specialist plays a dual role providing compassionate psychosocial support to hospice patients and families while also supporting employee engagement, retention, and workforce well-being initiatives. This position delivers social work services in accordance with hospice regulations and interdisciplinary care standards, while also serving as a resource for staff support programs, employee engagement initiatives, and organizational morale-building efforts. The role requires the ability to balance patient-facing responsibilities with employee-focused support, maintain confidentiality, and collaborate effectively with clinical teams, Human Resources, and leadership to enhance both patient care and employee experience.

Requirements

  • Bachelor’s degree in Social Work required; Master’s degree preferred in accordance with hospice standards.
  • Current Social Work licensure consistent with state and hospice regulatory requirements.
  • Experience in hospice, healthcare, or human services preferred.
  • Ability to balance dual responsibilities while maintaining role clarity and regulatory compliance.
  • Strong understanding of hospice philosophy, psychosocial care, and interdisciplinary collaboration.
  • Knowledge of employee engagement, retention, and support strategies in healthcare settings preferred.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to exercise discretion, professionalism, and sound judgment.
  • Creative problem‑solving skills with a focus on low‑cost, high‑impact employee support initiatives.
  • Strong documentation skills and attention to detail.
  • Must be able to travel as require
