About The Position

We’re seeking a compassionate and dynamic Social Services Manager to join our team. About the Role The Social Services Manager plays a key role in enhancing residents’ emotional and psychosocial well-being by acting as their advocate. The Social Services Manager protects vulnerable residents and ensures that their best interest is observed and helps them to find remedies to their situation. You will work collaboratively with nursing, therapy, and administrative teams to coordinate care plans, ensure regulatory compliance, and foster a supportive, home-like environment for residents and families.

Requirements

  • Bachelor’s degree in Social Work or related field required.
  • Knowledge of state and federal regulations governing skilled nursing facilities.
  • Strong communication, leadership, and organizational skills.
  • Passion for serving seniors and supporting quality of life.
  • All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per each state’s specific background check requirements prior to contact with patients/residents.

Nice To Haves

  • Experience in post-acute/ skilled nursing preferred.
  • Bachelor’s or Master’s degree in social work or related field preferred.

Responsibilities

  • Oversee social services operations within the skilled nursing facility.
  • Conduct assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
  • Provide counseling and support to residents and families regarding transitions, discharge planning, and resources.
  • Ensure compliance with state, federal, and CMS regulations related to social services.
  • Lead and mentor social services staff while promoting teamwork and collaboration across departments.
  • Build strong relationships with residents, families, and community partners to ensure continuity of care.
