Scheduled Weekly Hours: 40 Emplify Health by Gundersen Health System is seeking a full-time Social Services Liaison. This is your opportunity to be a part of a great organization who priorities patient care! What you will do: Work 40 hours/week Monday-Friday, no weekends or holidays Work remotely Must live in WI, MN or IA Partner on special projects and duties to assist our patients and their discharge needs. Requirements
- Bachelor's degree in Social Work or in a related field
- 1 year of previous experience in human services or health care
- Must live in WI, MN or IA
