About The Position

An established skilled nursing facility is seeking an experienced Social Services Director to oversee and coordinate resident-centered social service programs. The ideal candidate will bring compassion, leadership, and a solid understanding of long-term care regulations to ensure the emotional and psychosocial well-being of residents.

Requirements

• Bachelor’s Degree in Social Work, Psychology, Sociology, or related field (Master’s preferred).
• Minimum of 2 years of experience in social services within a skilled nursing or long-term care environment.
• Strong knowledge of Medicare/Medicaid regulations and discharge planning.
• Excellent communication, leadership, and organizational skills.
• State licensure or certification as required.

Responsibilities

• Direct and manage all aspects of the social services department.
• Conduct psychosocial assessments and develop individualized care plans.
• Provide counseling and support for residents and families.
• Coordinate discharge planning and collaborate with community resources.

• Maintain accurate and compliant documentation.
• Supervise and train social services staff.
