About The Position

Social Services Director Bear Creek Nursing & Rehab Center – Full Time Monday–Friday | Routine Schedule | Occasional Weekends Bear Creek Nursing & Rehab Center is seeking a compassionate, dedicated, and experienced Social Services Director to join our leadership team. This role is essential in supporting our residents and families, promoting emotional well-being, coordinating essential services, and ensuring a smooth transition throughout the continuum of care. Position Overview The Social Services Director is responsible for planning, coordinating, and managing the social service programs within our skilled nursing facility. This position plays a vital role in advocating for residents, supporting families, assisting with admissions and discharges, and ensuring psychosocial needs are met in accordance with federal, state, and facility regulations. This is a full-time Monday–Friday position , with occasional weekend responsibilities based on resident or family needs.

Requirements

  • BSW or MSW preferred
  • Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience required
  • Strong understanding of psychosocial assessments, care planning, and regulatory requirements
  • Excellent communication, customer service, and documentation skills
  • Ability to establish trust and rapport with residents, families, and staff
  • Strong organizational skills with the ability to manage multiple priorities
  • Compassionate, patient, and professional demeanor

Responsibilities

  • Provide support to residents and families during admission, stay, and discharge
  • Complete psychosocial assessments, care plans, progress notes, and required documentation
  • Participate in interdisciplinary team (IDT) meetings and collaborate with nursing, therapy, and administrative leadership
  • Serve as an advocate for residents’ rights, emotional well-being, and social needs
  • Assist residents and families with community resources, financial processes, and long-term planning
  • Coordinate room changes, care conferences, and behavioral support as needed
  • Support residents through transitions, grief, concerns, and psychosocial challenges
  • Ensure compliance with federal and state guidelines for social services in skilled nursing
  • Provide staff education related to resident rights, communication, and behavioral considerations
  • Maintain strong relationships with residents, families, team members, and outside agencies
  • Perform other duties as assigned to meet resident and facility needs
