About The Position

Social Services Director (BSW Required) Bear Creek Nursing & Rehab Center 8041 State Road 52 | Hudson, FL 34667 Full-Time | Monday–Friday | Skilled Nursing Facility Lead with Compassion. Advocate with Purpose. Bear Creek Nursing & Rehab Center is seeking an experienced and compassionate Social Services Director to join our leadership team. This is a key position within our skilled nursing facility, responsible for overseeing all social services programming and ensuring the psychosocial well-being of our residents. We are looking for a mission-driven professional with Skilled Nursing Facility (SNF) / Long-Term Care (LTC) experience and a strong clinical understanding of regulatory standards and resident advocacy. This is a full-time, Monday–Friday position , with occasional weekend responsibilities based on resident or family needs. Position Overview The Social Services Director plans, coordinates, and manages the social service programs within the facility. This role serves as a primary advocate for residents and families, ensuring emotional support, smooth care transitions, and compliance with federal and state regulations. This position plays a critical role in admissions, discharge planning, care conferences, behavioral support, and maintaining resident rights.

Requirements

  • Bachelor of Social Work (BSW) required
  • Skilled Nursing Facility (SNF) / Long-Term Care (LTC) experience required
  • Strong knowledge of psychosocial assessments, care planning, and regulatory compliance
  • Excellent documentation and communication skills
  • Ability to build trust with residents, families, and team members
  • Strong organizational and time-management abilities
  • Compassionate, professional, and resident-centered approach

Responsibilities

  • Conduct psychosocial assessments, develop care plans, and complete required documentation
  • Provide support to residents and families throughout admission, stay, and discharge
  • Lead discharge planning and coordinate transitions of care
  • Participate in interdisciplinary team (IDT) meetings and collaborate with nursing, therapy, and administrative leadership
  • Advocate for resident rights, dignity, and emotional well-being
  • Assist families with community resources, financial processes, and long-term planning
  • Coordinate care conferences and behavioral interventions as needed
  • Ensure compliance with federal and state regulations governing social services in skilled nursing
  • Provide education to staff regarding communication, behavioral considerations, and resident rights
  • Build strong relationships with residents, families, and community agencies
