Major emphasis will be placed on development and oversight of behavioral health and Alzheimer/Memory care communities.

Requirements

  • Knowledgeable of, and able to operationalize rules and regulations governing patient care and treatment related to Social Services.
  • Able to effectively communicate and present information in a clear and concise manner both orally and in writing to individuals/groups of varying educational backgrounds.
  • Expert knowledge in the areas of social services, activity programs and transitions of care; Able to establish and maintain effective working relationships with associates, department heads, administrators, and health service teams to promote positive outcomes.
  • Communicates well with other associates, patients, family members, and visitors providing warm and friendly greeting and an approachable attitude.
  • Bachelor of Science Degree in Social Services or related field.
  • Minimum, two (2) years of experience in a supervisory capacity in a long term care center;
  • Possess a working knowledge of long-term care operational standards set forth in the Federal Register, Requirements of Participation.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or associates of the organization.
  • Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.
  • The ability to deal with problems involving several concrete variables in standardized situations.
  • This job requires proficiency with the following computer applications: Microsoft Outlook Microsoft Excel Microsoft Word
  • BS Degree in Social Services or related field.

Responsibilities

  • Assists with developing, implementing, maintaining and periodically updating written procedural guidelines relating to Social Services for skilled nursing centers.
  • Responsible for consulting with skilled nursing center Social Services associates
  • Assists in developing and implementing of specialty behavior communities and programming.
  • Conducts center visits to evaluate programs and effectiveness of social services provided.
  • Establishes and maintains effective inter-departmental communications to provide positive working relationships.
  • Participates in meetings and education for skilled nursing center Social Services Directors.
  • Provides regulatory support related to assigned areas of responsibility.
  • Assists in the development of in-service training material pertinent to areas of responsibility.
  • Provides assistance with on-site training/orientation with Directors as requested.
  • Assists with development of procedural guidelines.
  • Works collaboratively with the Director of Life Enrichment Services to develop and implement programming to support psychosocial patient needs.
  • Actively participates in Performance Improvement efforts.
  • Serves on corporate compliance committee, as requested.
  • Coordinates psychiatric and psychological activities with Physician Liaison.
  • Periodically reviews/updates admission practices to ensure compliance with federally mandated regulations.
  • Provides input to the Vice President of Social &Life Enrichment Programs for Inpatient Services as needed regarding personnel in the skilled nursing centers.
  • Provides on-site and periodic group training for clinical associates in the skilled nursing centers.
  • Coordinates/provides documentation of training.
  • Participates in Quality Assurance Reviews and identifies performance improvement opportunities and assists with Quality Assurance/Performance Improvement plan.
  • Participates in various committees and/or projects as requested.
  • Keeps abreast of changes in state and federal survey guidelines, Joint Commission standards and survey process and regulations by attending state and national meetings/seminars as needed.
  • Attends and participates in workshops, seminars, etc., to keep abreast of current changes in my field, as well as to maintain a professional status.
  • Honors patients’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
  • Complies with Compliance, Privacy and Security programs.
  • Reports occupational exposures to blood, body fluids, infectious materials and hazardous chemicals in accordance with the organization’s procedural guidelines governing accidents/incidents.
  • Follows established safety regulations when performing job tasks, to include fire protection/prevention, smoking regulations, infection control, etc.
  • Responds to expressed concerns while displaying a helpful, caring demeanor, answers questions when appropriate in a professional manner.
  • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
  • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
  • Performs other duties as necessary to ensure the success of the System.
