About The Position

Social Services Director Job Responsibilities: Responsible for delivery of social services to residents and families Handle pre-admission and admission interviews, informs resident and family of available services and resident rights and responsibilities Prepare social histories for use by professional facility staff and participates in resident care conferences Handle problems of adjustment to the long term care facility environment, maintains frequent contact with residents and families and serves as an advocate for patients Establish working relationships with community health, welfare, and volunteer groups Assist with arrangements, if needed, for out of facility health care, transportation, and counseling services Assist residents and families with transitions in and out of the facility, connecting them with needed services Maintain accurate records, ensuring compliance and proper tracking of resident progress and services

Requirements

  • Bachelor's Degree in Social Work and at least one year's experience in Long Term Care
  • Strong communication and interpersonal skills
  • Understanding of healthcare regulations and resources
