About The Position

Social Services Director – Skilled Nursing Facility Location: Orange, NJ A skilled nursing facility in Orange, NJ is seeking an experienced Social Services Director to lead and oversee all psychosocial services within the facility. This is a leadership role requiring prior director-level experience in a skilled nursing or long-term care setting , with direct responsibility for compliance, discharge planning, and resident advocacy. This position is suited for a professional who has managed social services operations, understands regulatory expectations, and can function as part of the facility’s leadership team.

Requirements

  • Minimum 2+ years of experience as a Social Services Director in a skilled nursing facility, long-term care, or rehabilitation setting
  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field (Master’s preferred)
  • Proven experience leading discharge planning and resident advocacy at the director level
  • Strong knowledge of CMS requirements, resident rights, and survey readiness
  • Experience working with Medicare, Medicaid, and community placement resources
  • Strong leadership, communication, and organizational skills

Responsibilities

  • Direct and manage all Social Services operations within the skilled nursing facility
  • Supervise social services staff and oversee daily departmental workflow
  • Ensure compliance with federal, state, and New Jersey long-term care regulations
  • Lead discharge planning processes and coordinate with hospitals, families, and community agencies
  • Serve as a leadership resource for residents’ rights, grievances, and care concerns
  • Participate in care plan meetings, interdisciplinary rounds, QAPI, and survey preparation
  • Maintain accurate, compliant documentation and department records
  • Collaborate closely with Administration, Nursing, Admissions, and Therapy leadership
  • Act as a point of escalation for complex family or psychosocial issues
