About The Position

Cheyenne Co Village is a trusted long-term care and assisted living community dedicated to providing compassionate, person-centered care for our residents. We strive to create a supportive environment that promotes dignity, independence, and quality of life. We are seeking a Social Services Designee to join our team. This role is essential in supporting the emotional, social, and psychological well-being of our residents. The Social Services Designee will serve as an advocate for residents and families, ensuring their needs are met and their voices are heard.

Requirements

High school diploma or equivalent required; additional education in social services or healthcare preferred.
Completion of Social Services Designee certification (or willingness to obtain).
Strong communication, organizational, and interpersonal skills.
Experience in long-term care or assisted living setting preferred.
Knowledge of state and federal regulations related to resident care.

Responsibilities

Assist residents and families with adjustment to long-term care and assisted living environments.
Provide support during admissions, transfers, and discharges.
Maintain accurate documentation in compliance with state and federal regulations.
Coordinate care planning and participate in interdisciplinary team meetings.
Address psychosocial needs, including counseling and resource referrals.
Advocate for residents’ rights and ensure a respectful, caring environment.
