About The Position

Summary: The Social Service Coordinator provides medically related social services to attain or maintain the highest practicable physical, mental, and psycho-social well-being of each resident and provides direct services as needed. Essential Duties and Responsibilities: 1. Completes and distributes all necessary paperwork for resident admissions and resident discharges 2. Conducts resident assessments 3. Initiates and participates in resident care plans and other family meetings 4. Collaborates with other team members to address resident and family concerns 5. Documents in residents’ charts regarding pertinent information to conform with EverTrue policy and state and federal regulations 6. Identifies residents in need of services either through direct referral from families, other residents or staff, community incident reports, trigger reports and/or assessments and provides follow-up as needed 7. Communicates effectively with home health agencies, other nursing homes, hospitals, etc, regarding residents 8. May be assigned to conduct closings in AL/ALMC/Personal Care and Care Center 9. Assists in supporting other departments as needed to benefit residents and families

Requirements

  • Ability to communicate verbally with a high degree of sensitivity.
  • Strong written communication skills and documentation skills.
  • General understanding and knowledge of geriatrics
  • Must be able to adjust to daily interruptions and re-arrange schedule due to resident, family, and community needs.
  • Ability to function independently and in a team environment.

Nice To Haves

  • A Bachelor of Social Work, Gerontology, or related degree preferred
  • Prior experience working in long term care preferred
