About The Position

The Social Services Coordinator is responsible for providing Social Services interventions which successfully address residents’ needs and to link social supports, physical care and physical environment with residents’ needs and individuality in accordance with all state and federal regulations.

Requirements

  • Must be a graduate of an accredited college or university with a major in social services.
  • Must have a current license to practice as a social worker in the State of Ohio.
  • At minimum 2-years’ experience as a social worker in a health care setting preferred
  • Excellent problem-solving skills and ability to multi-task and prioritize tasks
  • Excellent customer service and communication skills
  • Ability to maintain required documentation
  • Ability to follow written and verbal instructions and to work independently and as a part of a team
  • Ability to remain calm and cope with the mental and emotional stress of the position
  • Must be in good general health and free of communicable diseases
  • Must have visual acuity and ability to hear, with or without use of prosthetics
  • Must have the physical ability to sit, stand, walk, bend, and move intermittently throughout the shift, as well as the ability to climb and descend stairs, use elevator and assist residents
  • Must have the ability to read, write and speak the English language

Responsibilities

  • Communicate in a positive manner with residents and their family/responsible party in the resolution of problems, to provide verbal counselling, and to communicate resident’s physical, mental and psychosocial needs to family member/responsible party, staff members, consultants and/or other professionals
  • Complete and maintain a comprehensive assessment including social history, a plan of care and progress notes for each resident as required by the facility and state and federal regulations
  • Maintain individual residents’ rights for privacy, treatment schedules, property, civil rights and all other rights as designated by state and/or federal law.
  • Maintain confidentiality about residents’ past and present care and/or other information
  • Communicate in a positive manner with healthcare team members, residents, families, visitors and vendors
  • Adhere to facility policies and procedures, including infection control, safety, and HIPAA regulations
  • Responsible for maintaining a calm, supportive and caring environment for residents and staff
  • Responsible for complying with all safety and sanitation procedures including universal precautions
  • Responsible for attending in-service programs, educational workshops and meetings as required by the facility and state
  • Perform other duties as assigned
