The Social Services Coordinator is responsible for the evaluation and assessment of resident's social service needs and concerns. Develops relationship with residents and families and ensures that all needs and concerns are met within a timely manner. Initiates Social Service charting and development of an activity program to meet resident's physical and mental stimulation needs upon admission and continues to monitor residents quarterly and completes charting as required. Demonstrates accountability for assessing, updating, and maintaining resident's rights through observation and assessment. Completes quarterly progress notes, social service log and participates in Resident Care Plan meetings.
