We’re seeking a dedicated and empathetic Social Services Coordinator to support the emotional and social well-being of our residents. This role is ideal for someone who thrives on building relationships, guiding life transitions, and creating programs that foster connection and quality of life.
Requirements
- Bachelor’s degree in social work, counseling, or related field
- Social Work license or eligibility to obtain one within 6 months
- Experience in senior living, CCRC, or similar setting preferred
- Strong communication, empathy, and organizational skills
- Proficiency in Microsoft Office and basic computer systems

Responsibilities
- Provide one-on-one emotional support and counseling to residents and families
- Assess and document residents’ social and emotional wellness
- Coordinate care plans and collaborate with internal teams and external providers
- Lead signature programs like New Resident Orientation and Continuum of Care Education
- Monitor residents for signs of isolation or decline and coordinate interventions
- Educate residents and staff on wellness, safety, and resident rights
- Maintain accurate documentation and uphold HIPAA standards
