About The Position
Absolut Care of Aurora Park is seeking a Social Worker to join our Long Term Care team in East Aurora, NY . This role is key in supporting residents and families while ensuring compliance with state and federal LTC regulations.
Requirements
- Degree in Social Work
- Knowledge of NYS DOH and federal LTC regulations
- Strong communication, documentation, and advocacy skills
Nice To Haves
- Experience in Long Term Care / Skilled Nursing strongly preferred
Responsibilities
- Provide psychosocial assessments and ongoing support for LTC residents
- Assist residents and families with care planning, adjustment, and discharge needs
- Coordinate with interdisciplinary team, nursing, admissions, and community resources
- Support care conferences, resident rights, and regulatory documentation
- Advocate for residents’ emotional, social, and psychosocial well-being
