About The Position

The Social Service Assistant is responsible for identifying the medically related psychosocial needs of residents and providing services to meet those needs. The Social Service Assistant will address residents' intellectual, developmental, emotional, spiritual and social functioning so that each resident can attain or maintain the highest level of psycho-social functioning and well-being pos s ible. F amilie s will also be offered services in that their positive and continued involvement may be critical to the residents’ quality of life. The Belmont Health and Rehabilitation is seeking a compassionate Social Service Assistant to become a part of our team where our primary goal is to provide exceptional care for our residents. About the Role: The Belmont Health and Rehabilitation is seeking a Social Services Assistant to- Assist in the entire admissions process from pre-admissions (pre-screening, tours, etc.) to discharge (placement arrangements, homecare referrals, etc.) Provide social services in accordance with accepted facility, professional and governmental regulatory standards. Act as a resident advocate and liaison with facility staff and with community resources as necessary. Develop, as neede d, and participate in the implementation of an appropriate B eha vior Management Program for applicable residents

Requirements

  • Indiana board certification in social work under IC 25-23.6-5-1
  • 1+ years’ experience in health care setting working directly with individuals
  • Bachelor’s or advanced degree in social work or a human services field

Responsibilities

  • Assist in the entire admissions process from pre-admissions (pre-screening, tours, etc.) to discharge (placement arrangements, homecare referrals, etc.)
  • Provide social services in accordance with accepted facility, professional and governmental regulatory standards.
  • Act as a resident advocate and liaison with facility staff and with community resources as necessary.
  • Develop, as needed, and participate in the implementation of an appropriate Behavior Management Program for applicable residents
