About The Position

The successful candidate will be expected to: Serve as the first point of contact for referrals, conducting screenings and eligibility assessments using standardized tools (e.g., Units US platform). Identify and address social needs such as housing, food, security, transportation, and emotional well-being. Maintain accurate documentation of all outreach, screenings, and referrals; reports trends to support continuous improvement. Facilitate access to appropriate community and health resources, ensuring timely outreach, referral and follow-up. Participate in reviews, trainings, and community events to strengthen coordination and professional growth. Assist with both internal and external audits.

Requirements

Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets and data management.
Basic knowledge of human services, Medicaid, disability awareness, understanding of the Independent Living philosophy.
Strong computer skills are required.
Strong verbal and written communication, attention to detail, problem solving, excellent organization, time management, and ability to work with a diverse population of individuals is required.
Must be able to work independently and initiate work in a fast-paced environment.
Excellent Customer Service skills required.
A Valid NYS Driver’s License is required.

A College Degree in health, social services, or a related field preferred
