The responsibilities of the Social Care Coordinator will include the following: Essential Functions: Review daily off platform screenings/self-screenings and connecting those with unmet needs to Navigators, responding to daily voicemails/emails that come in from Members or Providers. Provide basic technical support such as looking up information in ePACES and similar platforms, ensuring timely and effective assistance when technical difficulties are encountered. Extract data from Medicaid disenrollment file and disable insurance in UniteUs platform. Monitor and respond to communication(s) received from Members made via phone calls or from the Care Compass Collaborative website. Maintain and manage ticketing systems to track inquiries and ensure timely responses. Act as a liaison between the Social Care Network team and other departments, facilitating clear communication and collaboration. Assist Social Care Navigators with low level screenings and eligibility assessments, as needed. May assume additional responsibilities, as required. MUST Maintain HIPAA compliance at all times.
