Community Health Centers of America (CHCA) is seeking a highly organized and proactive SNF Clinic Lead (Medical Assistant) to oversee daily operations supporting our Skilled Nursing Facility (SNF) programs. This role serves as the central point of coordination between providers, facilities, credentialing, and internal teams, ensuring seamless onboarding, compliance, communication, and clinical workflow execution. This is an ideal role for someone who thrives in a fast-paced environment and can independently manage provider operations, documentation, and multi-facility coordination. This will be a full time position based at our Fair Oaks office. This role offers the opportunity to make a meaningful impact in a dynamic and rewarding environment as a motivated individual with a passion for excellence and a commitment to providing top-tier support. The regular work schedule will be Monday - Friday days.
