Reports to the DON, the MDS Coordinator (RN) is responsible for coordinating and overseeing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) 3.0 assessments, Care Area Assessments (CAAs), and interdisciplinary care planning in accordance with CMS, CDPH, and federal regulation §483.20. A Registered Nurse (RN) must coordinate each assessment with participation of the interdisciplinary team (IDT). The MDS Coordinator collaborates with nursing, therapy, social services, dietary, activities, and business office staff to ensure accurate and timely completion of assessments under OBRA and PDPM. This role ensures clinical documentation supports MDS coding, reimbursement accuracy, quality measures, and skilled level of care while maintaining compliance and supporting resident outcomes.
