Provides comprehensive medical services for patients in a program of home-based and clinic care designed to serve the needs of medically fragile patients with complex conditions. Patients (and their family members) will need well-coordinated medical care that addresses their personal goals for well-being and quality of life. Care will include primary care, urgent care and Hospital at Home care, as well as collaboration with nurse case managers, care coordinators, behavioral health specialists and consulting physicians and other healthcare professionals who comprise the patients care team. The Complete Care Medical Director and Clinic Lead physician lead the care teams in providing efficient and compassionate care. Presbyterian Medical Group seeks a Board Eligible / Board Certified Geriatric Medicine or Internal Medicine physician to join our established practice in Albuquerque, New Mexico. The primary responsibilities of the PMG Attending Provider include being the attending of record for assigned PMG patients in a designated Skilled Nursing Facility (SNF) and providing follow-up care up to 90 days post SNF in a PMG clinic or home setting. Secondary responsibilities include primary, urgent, and Hospital at Home care as part of a shared staffing model. Oversees and is accountable for the overall quality of care and clinical outcomes of patients seen. This care will be delivered by interdisciplinary team members to assure high quality patient care and outcomes. Health care providers focus on patient and family goals of care and quality of life. Manage acute and chronic diseases, including support for any care transitions. Serves as attending for patients admitted to the Hospital at Home and Post Acute Care program. Offers clinical support to APCs on the team providing care to patients. Works to assure patient satisfaction with care. Addresses needs around Advance Care Planning for patients.
