About The Position

Under the direction of the Manager of Respiratory Care Services, following established policies, procedures, and professional guidelines, the Sleep Navigator will promote effective care coordination for patients with chronic disease by establishing partnerships between patients, families, nurses, physicians, and other qualified healthcare providers and clinical disciplines. Stratifies sleep patient population according to risk to effectively and efficiently manage patients. Partner with the provider care team for successful preventative care visits to reduce the severity of chronic disease and avoid able acute illness related to sleep disorders. Provide effective clinical health coaching to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk. Establish resources within the organization to assist with co-morbidity related to obesity, tobacco cessation, diabetes prevention/management, depression, anxiety and managing chronic illness. Facilitate patient access to appropriate medical and specialty providers as indicated by physician or qualified healthcare provider. Determine frequency of need for provider appointments. Formulate and communicate goal directed plan of care with the patient. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital and principles of quality improvement.

Requirements

  • Bachelor of Science in Nursing
  • Licensure to practice as a Registered Nurse in the State of Florida
  • Minimum of 2 years’ work experience in a health care setting
  • Experience in chronic disease management required
  • Must have strong computer skills including Microsoft Windows, spreadsheets, databases and Web-based software

Nice To Haves

  • One year’s experience in outpatient/physician office preferred
  • Previous Care Coordination or Case Management experience preferred
