About The Position

The Division of Geriatrics & Gerontology, Department of Medicine, UW School of Medicine and Public Health is seeking applicants for a part-time assistant, associate or professor on the Clinician Teacher (CT) track. The successful candidate will join a world-class institution that encourages, values and supports interdisciplinary basic, clinical and population health research, and attracts scholars and students from around the world. The faculty member will provide primary care at various skilled nursing facilities. The candidate will provide clinical teaching to medical students, residents, and fellows. Madison is the state's capital city with a population of over 252,000, and offers an ideal combination of natural beauty, stimulating cultural events, outstanding schools and outdoor recreation. This position is full or part time, 20%-40% This position requires work to be completed onsite, at a designated campus work location. This position has been identified as a position of trust with access to vulnerable populations.  The selected candidate will be required to pass an initial caregiver check to be eligible for employment under the Wisconsin Caregiver Law and repeated every four years. Applicants for this position will be considered for the titles: Clinical Professor, Clinical Associate Professor. The title is determined by the experience and qualifications of the finalist. Candidates who demonstrate the following knowledge, skills, and abilities, will be given be given first consideration Key Job Responsibilities: Position responsibilities for patient care include predominantly primary care for older adult patients at skilled nursing facilities. Responsibilities will also include clinical teaching of medical students, residents and fellows, and other service appropriate to the position.

Requirements

  • WI medical license by start date of position.
  • Completion of a US internal medicine or family medicine by the start date of the position.
  • MD or DO, or equivalent is required

Responsibilities

  • Provide 8-10 hours of total SNF care time per week per 0.2 FTE.
  • Consistently meet minimum work expectations related to SNF resident face-to-face care 3-4 SNF residents per half day
  • Round 2 half days per week at SNFs in Dane County
  • Each 0.2 FTE position will cover 4-5 facilities, rounding at least once per month at each site
  • Schedule fill rates >90%
  • Address patient non face-to-face patient care needs in a timely fashion in collaboration with the APP team (includes telephone encounters, My Chart messages, etc).
  • Provide care in alignment with UW Health panel management principles and the quadruple aim

  • Participate in department/division and site activities as directed by Chair or Division Head
  • Participate in provision of clinical services during non-traditional hours as determined by your Department
  • Coordinate efforts of your care team so that needs of patients are assessed and managed by appropriate members of the care team when facility staff/patients/surrogate decision-makers contact your office during regular hours of operation.
  • Delegate coverage of your panel to Care Team members when you are unavailable and participate in coverage of the patients of other Care Team partners when they are unavailable.
  • Participate in needed call responsibilities defined by department/division or site.
  • Complete all needed medical record documentation in a timely way consistent with ambulatory standards.
  • Provide a minimum of 44 weeks of clinical care per year (accounts for up to 22 days vacation, 2 weeks CME, and official weekday holidays when offices are closed).
  • Participate in teaching of medical students, residents and other learners within your clinic setting.
  • Participate in site/division/department and organizational efforts that focus on improvement of care and service provided to your patients.
  • Be accountable to achieving quality of care and Ambulatory Care Service Goals and Standards.
  • Establish and maintain a relationship with a skilled nursing facility (SNF) panel of at least 60 long term care residents per 0.2 FTE appointment.
  • Direct and coordinate provision of the essential elements of health care services for the panel, including:
  • Completion of required in-person compliance visits per CMS regulations at the resident's facility
  • Recommendations for medical and behavioral screening and prevention activities and services
  • Chronic disease management
  • Coordination of care with extended members of the Care Team, including UW Health SNF advanced practice providers (APPs) and nurse case managers, UW Health SNF pharmacist, SNF medical directors, SNF clinicians, subspecialists, ER and Urgent Care clinicians, hospitalists, hospice clinicians and other non-physician providers
  • Communication with surrogate decision makers when appropriate and/or involved family members per SNF resident preference
  • Discussion and documentation of care preferences
  • Completion of advanced care planning documents
  • End of life care
  • Maintain the electronic health record for your patient panel:
  • Problem lists
  • Past medical, surgical, social and family history
  • Medications
  • Health Maintenance
  • Participate in site/division/department and organizational activities as directed by your Chair (this time is to be considered to part of your total clinic time) (see below).
  • Utilize specialty physician partners within UWMF for needed care outside of your Care Team -- exceptions based on specific payer requirements.
  • Maintain board certification.
  • Participate in an annual performance review.
  • Provide leadership and direction for advanced practice providers (APPs), learners and staff members of your local care team.
  • Direct the allocation of work to the most appropriate skill level of the team members.
