About The Position

The Skilled Nursing Facility (SNF) Facilitator promotes high quality, patient-centered care by facilitating patient placement to skilled nursing facilities, identifies and removes barriers to SNF discharges. The SNF facilitator acts as a liaison between the hospital, SNFs and payors. Coordinates prompt SNF acceptance and facilitates payor authorizations where applicable. SNF Facilitator communicates effectively daily with RN Case Managers, Social Workers and Case Management Leadership.

Requirements

  • Minimum of two years' experience in payor or acute care discharge planning and/or skilled nursing discharge planning experience required.
  • Understanding of payor guidelines and SNF placement process
  • Associate's Degree-Major: Nursing
  • Bachelor's Degree-Major: Nursing
  • Currently licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.
  • FT/PT/PRN colleagues with a compact license from a state other than Texas must:
  • apply for Texas RN license within 60 days from hire
  • obtain current TX RN license within 6 months of application for TX RN license
  • Required Upon Hire
  • BCLS Required Upon Hire
