We are seeking a knowledgeable, organized, and mission‑driven Service Plan Specialist to support high‑quality, person‑centered service planning across Community Services. This role plays a key part in ensuring services are thoughtfully designed, compliant with Medicaid and regulatory requirements, and responsive to the unique needs, goals, and strengths of each person served. Requirements

  • Bachelor’s degree in human services, social work, psychology, or a related field . A combination of education and experience will be considered.
  • Four (4) years of experience in Medicaid service delivery .
  • Ability to travel for meetings and appointments held in the community; primary work location is office‑based with hybrid work model potential.
  • Knowledge of Medicaid requirements, Iowa Administrative Code, and accreditation standards.
  • Strong organizational, analytical, and communication skills.
  • Ability to manage multiple priorities while maintaining attention to detail and accuracy.
  • Proficiency with computers and Microsoft Office applications, with Excel preferred.
  • Valid driver’s license, reliable vehicle for on-the-job use, current vehicle insurance, acceptable driving record, and ability to pass required background checks and drug screening.

Nice To Haves

  • Strong preference for experience with functional assessments and service plan development.

Responsibilities

  • Develop, write, review, and revise Individualized Service Plans (ISPs) that are person‑centered, compliant, and reflective of each individual’s goals, needs, and preferences.
  • Conduct and analyze comprehensive and functional assessments, including health risk screening tools (HRST), and ensure significant changes are accurately reflected in service plans.
  • Coordinate and facilitate annual and additional service planning meetings with individuals served in our community and interdisciplinary team members.
  • Ensure service plans align with Iowa Administrative Code, funder requirements, authorizing plans, and accreditation standards.
  • Support quality assurance by monitoring documentation accuracy, timelines, and regulatory compliance.
  • Provide technical assistance, coaching, and training for Community Services leadership and staff related to service planning standards and best practices.
  • Collaborate closely with the Quality Improvement & Outcomes Director to promote consistent quality outcomes, and continuous improvement.
  • Communicate professionally using appropriate clinical language regarding diagnosis, behavioral health needs, and service recommendations.
  • Participate in community‑based meetings, often occurring in homes, provider offices, and other settings.
