About The Position

The Service Navigator plays a crucial role in supporting clients to develop personalized plans for their desired quality of life within the scope of LOGAN's services. This involves facilitating a comprehensive understanding of the array of services offered by LOGAN and working closely with clients to identify an individualized combination of services that align with their interests, abilities, and needs. Service Navigators collaborate with teams to overcome obstacles hindering desired service outcomes, drawing upon LOGAN services, community resources, and innovative ideas. Continuous communication with clients is necessary to assess service satisfaction and make modifications when needed. The Service Navigator collaborates well with other departments to ensure seamless service delivery that supports vocational, social, and integrated outcomes.

Requirements

Bachelor’s degree in social services, Education, or related area preferred but not required.
A minimum of five years’ experience with individuals with intellectual and developmental disabilities is required.
Demonstrated ability to develop and implement engaging, creative, learning curriculum.
Demonstrated ability to relate effectively with people who have disabilities.
Demonstrated ability to communicate effectively with coworkers and supervisors.
Ability to communicate effectively, both verbally and in writing.
Ability to interact effectively with a variety of personalities, including clients, parents, families, guardians, and other providers.
Ability to organize information and materials.
Ability to prioritize assignments and meet deadlines.
Ability to work independently and use sound judgment.
Requires a high degree of tact, honesty, confidentiality, maturity, and attention to detail.

Responsibilities

Supports clients on assigned caseload to develop their plan for a meaningful life as related to their services at LOGAN so that they can identify and develop their potential.
Follows through promptly with new referrals as assigned, assisting them in navigating LOGAN services.
Is knowledgeable and shares the array of services available at LOGAN with clients and assists in determining an individualized combination of services that fit each person’s interests, abilities, and needs.
Works with clients and their team to develop a Positive Personal Profile (PPP), which will be used to identify which Pathway in Life Enrichment and Employment Pathways is initially chosen.
In collaboration with the client and their team, updates the Positive Personal Profile (PPP) as interests and desires change because of lived experiences.
Consistently communicates with clients to assess service satisfaction and modify service plan and individual schedule as needed.
Assists teams with finding solutions to obstacles to desired service outcomes. These solutions may include LOGAN services, community resources, or other ideas.
Communicates service staff training needs to Service Managers and Leads as necessary to support individualized service delivery and creative problem-solving.
Supports collaboration between departments at LOGAN to provide seamless service delivery that supports desired vocational, social, and integrated outcomes. Facilitates connections and communication.
Provides measurable outcome data as requested by Service Navigation Director reflecting clients’ individualized service choices.
Assesses the interests, skills, needs, and preferences of each client. Actively listens to clients and encourages them to express their personal preferences and interests. Observes clients to ascertain their interests if they are not readily able to articulate them.
Promotes a department culture centered around an employment-first mindset.
Demonstrates familiarity with the Vocational Rehabilitation Services procedure, covering aspects from initial referral to successful closure.
Completes tracking forms related to productivity and service efficiency as requested by Service Navigation Director.
Represents all LOGAN departments well to participant teams and other stakeholders.
Assists in promoting the health and safety of individuals served by LOGAN, visitors, and other staff.
Serves as a role model for individuals served and other staff.
Serves as an advocate and protects the human rights of all individuals served.
Participates in training as required by supervisor.
Facilitates professional growth by keeping current on new philosophy, research, technology, program strategies, etc.
Serves on internal and external committees as assigned.
Completes other duties as assigned by supervisor.
