The Service Line Clinical Coordinator provides specialized coordination of care for multiple Center of Excellence (COE) patient populations, including Orthopedic, Spine and Cardiovascular. Accountabilities include pre-op patient education, care redesign and facilitation between Orthopedic COE, Spine COE and Cardiology COE providers and the hospital. Responsible for the patient education program which includes the development, revision, and implementation of the patient education program materials, and resources in the specialty area. Assist with performance improvement, program development, outcomes data collection and analysis, and participation in ongoing multidisciplinary clinical team meetings. Serve as the clinical liaison between the hospital and physician practice(s) ensuring an excellent patient experience with a focus on patient satisfaction and quality. Perform duties where competent in any clinical capacity and other duties as assigned. Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, and photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Word, PowerPoint, internet, and e-mail) required. Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.
