About The Position

If you have a passion for organization and a heart for advocating with individuals who have intellectual and developmental disabilities, we have an opportunity for you. Service Coordinators with HeartWell Services act as the primary liaison for individuals with developmental disabilities, facilitating the person-centered planning process. They coordinate care, ensure individual health and safety, develop individual service plans, monitor service delivery for safety and efficacy, conduct regular in-person meetings, and ensure compliance with state regulations. They are the heart that holds their caseload together. We have opportunities in both residential-based service coordination and community-based service coordination. Job details: This is an exempt salaried position with an on-call rotation. Residential- based service coordinators may need to cover shifts within the service home, which could include graveyard shifts. Community- based service coordinators may need to cover shifts within the field and may include weekends on occasion.

Requirements

  • Experience working with people who have developmental disabilities.
  • Strong organization, communication, and problem-solving skills.
  • Ability to self-direct and work independently.
  • Unencumbered Driver’s license
  • Current vehicle registration and insurance
  • A personal vehicle
  • Be able to pass the background check
  • Be able to pass the drug test
  • Willing to work shifts
  • Participating in an on-call rotation
  • Willingness to commute

Nice To Haves

  • Previous experience within service coordination preferred or in case management.

Responsibilities

  • Person-Centered Planning: Developing and updating each individual service plan to reflect the person’s goals, wishes, and needed support. Advocate for the individual’s likes and dislikes.
  • Monitoring and Compliance: Ensure providers (DSP’s) adhere to DDSD Waiver standards, company policies, individuals’ plans and supports, reviewing service effectiveness, and conducting regular in-person visits.
  • Collaboration: Coordinating with the individual's entire team (IDT), to ensure the needs and wants of each individual are heard and supported.
  • Documentation and Reporting: Maintaining accurate records both internally and within online systems. Utilize a document matrix to ensure compliance and ease of locating documents. Keep current on all incident reports, annual and bi-annual reports, and any reports as required by DDSD Standards.
  • Advocacy: Ensuring individuals have access to community life, employment-first opportunities, and protection of rights.
  • Safety & Welfare: Ensuring the health and safety of individuals is the primary focus of service coordination.
  • Accountability: Acting with integrity, honoring commitments, and promoting transparency in service delivery within the agency as well as with outside service providers and team members.
  • Independence: Promoting personal choice regarding where to live, work, and socialize.
