Title: Service Coordinator
Company/Location: Kentucky River Community Care/Can be based in either Breathitt, Lee, Owsley or Wolfe Counties
Employment type: Regular Full Time

Requirements

• Bachelor's degree in approved Human Services field (Social Work, Psychology, Sociology, etc.) with one (1) year post graduate work with children.
• good communication skills
• maintain client related goal data, including case notes, outreach, and relationships with community partners.

Responsibilities

• Provide case management services including needs assessment, service plan and monthly contacts.
• Develop and coordinate community wide outreach for youth.
• Develop plan of intervention that address needs for housing, services, crisis and intervention.
• Link clients to available resources to assist with needs and progress; assist client with resolving identified barriers; monitor clients' needs and progress, coordinate and facilitate with treatment team to assist client with reaching person centered goals.
• Network with agencies, coalitions, and local community meetings.
