What You’ll Do: As a Service Coordinator, you’ll provide case management and individualized supports to adults ages 18 through end of life. Our participants have diverse interests, abilities, and support needs, and you’ll play a key role in coordinating wraparound services that help them thrive in their communities. Requirements
- Reliable personal transportation is required
- Bachelor’s degree preferred, but we will consider an equivalent combination of related education, training, and experience
