About The Position

OVERVIEW Coordinates and manages the support coordination services system, providing case management, outreach, assessment, service linkage, monitoring, advocacy, and counseling for individuals with developmental disabilities. Work is performed under general supervision and is guided by agency policies and procedures, quality assurance, public and private payer guidelines, professional practices, and ethical standards related to client interactions, as well as state and federal licensure and accreditation regulations and guidelines. DUTIES AND RESPONSIBILITIES Oversees the operation of the program, including: staff recruitment and development, shift scheduling, supervision and evaluation; individuals screening and registration; individuals programming development and evaluation; program record-keeping and reporting; individuals and staff records maintenance; safety program policy and procedures; program health and safety; and program rules . Monitors individual documentation, programs, and facilities for compliance with program, agency, licensure, quality assurance, Medicaid, and other payor policy, standards, guidelines, and requirements. Collects, assembles, analyzes, and prepares statistical and other reports as necessary. Provides training and guidance and oversees the preparation and maintenance of client records. Assists in developing the program budget, monitors program expenditures and revenues, and identifies and communicates resource deficits. Serves on committees within the agency and with generic services as needed. Responds to, resolves, and/or refers to appropriate staff inquiries and complaints to the unit. Coordinates individual services and program services with agency treatment and other service providers. Supervises and participates in the preparation of statistical and individual reports. Attends various community, governmental, and other meetings regarding persons with developmental disabilities. Develops, recommends, monitors, and evaluates policies and procedures related to service coordination. Oversees input of service and other documentation to the electronic records system. Does related work as required.

Requirements

  • Extensive knowledge of the definition, causes, programs, and services relevant to the developmentally-disabled population
  • Extensive knowledge of individuals' rights
  • Extensive knowledge of local community resources and service and developmental delivery systems, including support services, eligibility criteria and intake processes, termination criteria and procedures, and generic community resources
  • Extensive knowledge of the different types and uses of assessments in developing support plans for individuals with developmental disabilities, including the Virginia Individual Developmental Disability Eligibility Survey
  • Extensive knowledge of the administration of the State-wide Waiting List and DD Waiver Supports for individuals with a developmental disability, including the WaMS System
  • Extensive knowledge of Person Centered thinking and Person Centered Practices
  • Thorough knowledge of the types of developmental disability programs, services, treatment modalities, and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning and service coordination

  • Ability to collect, compile, and evaluate electronically sourced or other sourced statistical and financial data and prepare complex recommendations, proposals, reports, etc.
  • Master's Degree in the Behavioral Sciences with extensive professional work experience involving case management services to the developmentally delayed and/or mentally ill population, including considerable program management and supervisory experience.
  • Valid Virginia driver's license
