In this role, you will support safe, timely transitions of care for patients moving from the Emergency Department to inpatient units, outpatient services, or home. You will partner with ED pharmacists, nurses, and other care team members to ensure patients have the medications, instructions, and follow‑up they need so their treatment continues smoothly and without interruption. Your work will center on real‑time support within the ED -helping prepare patients for discharge, coordinating medication access, facilitating care transitions, and tracking movement across care settings to maintain continuity of therapy. Role Highlights Coordinate transitions of care for ED patients, ensuring medications and therapy plans follow the patient to their next care setting. Support ED discharges by assisting with medication procurement, counseling coordination, and follow‑up arrangements. Partner with pharmacists, nurses, case management, and pharmacy financial counselors to reduce barriers to discharge. Assist with medication histories and support ED pharmacy workflows as needed. Follow policies and procedures that promote patient safety, efficiency, and high‑quality care during rapid patient throughput.
