The Senior Social Worker provides comprehensive social work services and case management to residents of The Hoy Center. This role involves coordinating bed management across Westminster-Canterbury’s Enhanced Services, The Hoy Center, and the broader community to ensure efficient transitions and continuity of care. The Senior Social Worker also oversees transfers within the Westminster-Canterbury population and collaborates on discharge planning for both residents and community members, ensuring safe, well-coordinated discharges with appropriate follow-up care. Additionally, the Senior Social Worker fosters a warm, supportive environment for residents and families, consistently modeling and upholding the organization’s core values of respect, passion, and professionalism.
