About The Position

The Senior Social Worker provides comprehensive social work services and case management to residents of The Hoy Center. This role coordinates bed management across Westminster-Canterbury’s Enhanced Services, The Hoy Center, and the broader community, ensuring efficient transitions and continuity of care. The Senior Social Worker oversees transfers within the Westminster-Canterbury population and collaborates on discharge planning for both residents and community members, ensuring all discharges are safe, well-coordinated, and supported with appropriate follow-up care. In addition, the Senior Social Worker fosters a warm, supportive environment that promotes personal connection and a calm atmosphere for residents and families. This individual consistently models the organization’s core values of respect, passion, and professionalism, while also holding others accountable to the same standards. Founded in 1982, Westminster-Canterbury on Chesapeake Bay is a not-for-profit, faith-based Life Plan Community located along the southern shore of Chesapeake Bay. The Hoy Center takes a holistic approach to skilled nursing care and aging, featuring home-like settings called Lighthouses with family rooms and e-concept kitchens, private rooms, outdoor terraces, and a prayer room.

Requirements

  • Bachelors' degree in Social Work or degree in Human Services.
  • Minimum of two years of social work experience in a long-term care setting.
  • Has familiarity with patient charts, medical terminology, and documentation procedures.
  • Familiarity with Microsoft applications and Google Suite, and knowledge in EMRs.
  • Flu-Vaccinated

  • Previous experience working with a senior living community.

Responsibilities

  • Establish relationships with residents and family members to serve as psycho-social liaison through all stages of residency.
  • Participate in developing and implementing individual resident care plans.
  • Organize and lead Interdisciplinary care planning meetings.
  • Perform shopping needs for residents in The Hoy Center.
  • Assist residents and family members with Medicaid eligibility and application process.
  • Maintain knowledge base of relative Medicaid and Medicare guidelines, documentation and program requirements.
  • Maintain social work documentation consistent with regulatory requirements, including completion of social histories, MDS assessments, and Medicare Notices of Non-Coverage.
  • Participate in meetings as assigned, including daily PDPM review, Resident Review meetings, bundled meetings.
  • Address and resolve resident concerns in a timely and professional manner.
  • Serve as a liaison with the interdisciplinary team for grievance follow up.
  • Participate in investigations, including Facility Reported Incidents and follow up interviews and action plans.
  • Develop and maintain strong relationships with bundled partners and manage communications with partners to promote resident quality of care.
  • Serve as the resident advocate in upholding Resident Rights and Responsibilities.
  • Serve as Administrator on Call for the campus for a one-week period.
  • Performs all other duties as assigned.
