The Senior Psychiatric Case Manager position is based in Pediatric Behavioral Health & Wellness which is a part of the clinical services of the Department of Psychiatry. The service provides outpatient behavioral health care to patients and their families from a wide geographic area. The Sr Psychiatric Case Manager will support ambulatory clinicians and the families they serve by providing consultation on community resources, preparing, submitting, and following up on referrals, and participating in appointments and community-based visits when clinically indicated. The Senior Psychiatric Case Manager must subscribe to UR Medicine ICARE values, including creating a culturally competent environment by treating patients, families, trainees, and co-workers with sensitivity and appropriate attention to cultural differences. The schedule for this position is generally Monday-Friday, 8:30 am-5 pm with occasional evening and weekend hours.

Requirements

  • Associate's degree in health-related field, 2-3 years’ experience as a psychiatric case manager; or an equivalent combination of education and experience required
  • 2-3 years’ experience as a psychiatric case manager; or an equivalent combination of education and experience required
  • Must possess a valid New York State Driver's License and access to automobile required

Nice To Haves

  • Lived experience working with the mental health system related to a family member preferred

Responsibilities

  • Provide consultation to clinicians regarding community resources for their patients and patients’ families
  • Meet with patients and/or caregivers to provide support and to assist with paperwork, correspondence, etc., necessary to ensure linkage to appropriate resources; as well as assist with access to urgently needed resources (e.g., food, housing, medical care).
  • For patients in acute crisis, collect information, prepare referrals, and follow-up on referrals to county-based OMH services, Health Homes, Preventive Services, Residential Treatment Facilities (RTF) and Residential Treatment Centers (RTC)
  • Responsible for documenting patient and family contacts and patient progress in the electronic medical record
  • Participate in evaluation and treatment appointments with mental health clinicians and patients/families as needed (e.g., to share information about community resources and programs)
  • Make community-based visits when clinically indicated to support youth and family advocacy efforts (e.g., school visits, CSE meetings, DHS)
  • Make collateral contacts for new and existing patients and families. Collateral contacts may include Child Protective Caseworkers, Foster Care Caseworkers, other DSS Caseworkers, probation officers, pediatricians and nurse practitioners, and school personnel.
  • Provide outpatient telephone screens to families seeking services for their child or adolescent.
  • Participate in designated meetings, e.g., monthly Child and Adolescent Outpatient Services Meeting, weekly Treatment Team Meetings, Patient Support Team Meeting; and other duties as assigned (e.g., updating community resource manual)
