Trillium Health Resources is a Tailored Plan and Managed Care Organization (MCO) serving 46 counties across North Carolina. We manage services for individuals with serious mental health needs, substance use disorders, traumatic brain injuries, and intellectual/development (IDD) disabilities. Trillium Health Resources has a career opening for a Senior Manager, Transition to Community Living (TCL) to join our team! The Senior Manager, TCL provides strategic oversight to ensure the integration of evidence-based clinical practices, adherence to DHHS and DOJ Settlement requirements, and consistent delivery of high-quality services supporting members transitioning into and sustaining residence in permanent supportive housing. This role also provides strategic leadership for internal clinical systems, quality improvement initiatives, and care coordination processes that promote effective transitional planning, continuity of care, and sustained housing stability.

Requirements

  • Master’s degree and three (3) years’ experience with behavioral health/mental health and substance use (BH/MH/SU) population; OR Equivalent combination of education/experience.
  • Valid North Carolina clinical license (LCMHC, LCSW, LMFT, or LPA) is required.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson.

Responsibilities

  • Provide clinical and administrative leadership TCL teams that report directly to them.
  • Directly supervise licensed and non-licensed managers and supplemental staff, ensuring accountability, performance, and professional development.
  • Monitor staff assignments and documentation through structured supervision and performance review processes.
  • Monitor and evaluate the teams activities, operational workflows, and financial procedures to ensure timely and effective service delivery.
  • Ensure discharge and transition planning occur efficiently and result in successful placements in permanent supportive housing for members identified the DOJ Special Health Care Population/TCL.
