About The Position

Under the supervision of the UCSF Primary Care Administrative Nurse Manager, the Licensed Vocational Nurse (LVN) utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socioeconomic background, and response to medical treatment. The LVN provides nursing care to pediatric and adult patients in an ambulatory primary care setting and serves as an integral member of the care team, working closely with Registered Nurses and physicians to deliver patient-centered care. Responsibilities include medication and vaccine administration, clinical procedures, patient education, and chart abstraction. This role requires flexibility to support multiple UCSF primary care clinic sites and a consistent commitment to professionalism, customer service, and UCSF values.

Requirements

  • Six months to two years of experience as a Licensed Vocational Nurse in an ambulatory setting
  • Current California Licensed Vocational Nurse (LVN) license
  • Current AHA CPR certification
  • Training and licensure to perform phlebotomy
  • Strong communication, problem-solving, and organizational skills
  • Ability to work effectively in a fast-paced clinical environment
  • Basic computer and keyboarding skills
  • Prior experience with electronic medical records
  • Active Licensed Vocational Nurse licensure in the State of California
  • Active American Heart Association CPR Certification

Nice To Haves

  • Bilingual language skills (Spanish, Chinese, or Russian preferred)
  • Familiarity with Patient-Centered Medical Home principles

Responsibilities

  • Patient Assessment & Care Planning Assess patient needs and responses to treatment through observation and clinical evaluation
  • Recognize and report physical or emotional changes to appropriate members of the care team
  • Participate in care planning using knowledge of physical, emotional, cultural, and socioeconomic factors
  • Identify appropriate resources to support patient care and education
  • Clinical Care & Procedures Provide individualized nursing care for patients across the lifespan (infant through geriatric)
  • Perform patient intake and support population health and quality initiatives
  • Administer medications and vaccines in compliance with clinical guidelines
  • Assist with and perform procedures including injections, wound care, ear irrigations, catheterization, phlebotomy, and point-of-care testing
  • Prepare patients for diagnostic and therapeutic procedures
  • Maintain exam room cleanliness, organization, and supply integrity
  • Documentation & Systems Document patient care, procedures, and responses accurately in the electronic medical record
  • Manage telephone messages and chart notes clearly and professionally
  • Utilize UCSF systems including ApeX and other computerized programs
  • Care Coordination & Communication Support efficient patient flow and continuity of care
  • Communicate effectively with patients, families, and the interdisciplinary care team
  • Manage assigned electronic in-basket tasks and cross-coverage as needed
  • Participate in daily team huddles to support patient-centered care
  • Patient Education & Advocacy Identify and document patient education needs
  • Provide health education using language appropriate for patient and family understanding
  • Utilize interpreter services and educational resources as appropriate
  • Support patient action planning and health coaching activities
  • Professional Practice & Improvement Participate in performance improvement and patient-centered medical home initiatives
  • Maintain compliance with infection control, safety, and regulatory standards
  • Complete required competencies, certifications, and ongoing education
  • Demonstrate professionalism, teamwork, and adherence to UCSF PRIDE values and AIDET communication standards
