About The Position

The Senior Director of Palliative Care and Hospice is a professional leader who, with direction from the Vice President of Post-Acute Care Services, assesses service operations in relation to compliance with legal, regulatory, and professional standards and organizes indicated operational adjustments. Responsible and accountable for the 24-hour operations of patient care in all areas of Palliative Care and Hospice, to include patient experience, personnel management, fiscal resources, performance improvement, and interdepartmental coordination / collaboration. Ensures standards of care are met and that relationship with physicians and all disciplines are collegial; working together to keep the patient and their family at the center of all decisions creating a Patients' First environment. Assumes responsibility for connecting Palliative and Hospice services to other PIH Health programs/services where the synergy and complementary nature of such services reach the goals and mission of PIH Health. Also accountable for the development and integration of social services programs for Post-Acute Care Services. Leads by example. Works with the team to reduce infrastructure and to promote high value performance. This is an exempt management position. Requirements

Sufficient background knowledge and expertise in clinical decision-making for the patient population to meet the needs of his or her patients, and to contribute to Quality Management review and evaluation.
Leadership and management skills; a demonstrated ability to coach and develop others in a Patients First perspective.
Effective written/oral communications
Interpersonal competence
Ability to promote team building among multidisciplinary team
Effectively delegates while holding those accountable to accomplish goals.
Must be able to manage changing priorities, problem solving, and organizational skills
Knowledge of Center for Medicare/Medicaid Services (CMS) Condition of Participation, Title 22 licensing requirements, and Center for Disease Control (CDC)
Knowledge in county and state regulations regarding unit specific standards and operations.
Knowledge of continuous quality improvement tools, Plan Do Check Act performance improvement methodology and techniques
Demonstrates commitment to PIH Health's philosophy of care, Mission, Vision, Values, and organizational culture.
Demonstrates knowledge and understanding of the End-of-life continuum and the benefit PIH Health provides within the integrated delivery system.
Demonstrated skills and experience in the provision, supervision, and management of various clinical and social services program.
Personal computer skills and knowledge of applicable software programs
Master's Degree in nursing, social work, or health care administration
Current BLS from the American Heart Association
Palliative and Hospice Certification required or to be obtained within two years of employment in this role
Minimum of three years' experience in a home health agency, primary care, or health facility
Experience within palliative care, hospice, social work, and chronic care management
Current California driver's license and valid auto insurance
