About The Position

Responsible for providing leadership and supervision of senior care coordinators in accordance with the plan of care based on a comprehensive health assessment and provides coordination /navigation of quality care services for members of ChooseHome. Manages health assessments, personal services, and program functions of the ChooseHome brand. Works under the supervision by the Executive Director of ChooseHome.

Requirements

  • Bachelors Degree, Degree of study in Nursing/Social Work or Related (Required)
  • 2 years Leadership Experience (Required)
  • 1 year 1 year Healthcare/Medical - Case Manager (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
  • Montreal Cognitive Assessment (MOCA) - MOCA Test Inc within 30 Days(Required)
  • Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
  • Valid Drivers License Required
  • Call Rotation

Responsibilities

  • Responsible for oversight of utilization goals for services and manage provider invoices and department budget.
  • Manages and ensures the provision of services to meet member needs and oversees Riverside ChooseHome Senior Care Navigation to include the performance and supervision of personal service coordinators to independently interpret baseline of member’s care and recommend appropriate services.
  • Leads personal service coordinators through changes in member’s level of care based on assessment and in conjunction with physician/provider notes as well as family communication and direct observation/home assessment.
  • Provides assessments for cognitive level and review of health conditions for new members entering the program.
  • Gives insight into admission eligibility to the medical director and executive director during admissions meeting for members entering the program through a health assessment.
  • Serves as the primary point of contact for an assigned panel of members, providing ongoing support through monthly check-in calls and annual in-home assessments.
  • Collaborates with family/primary providers, admissions coordinators, and discharge planners to ensure quality transitions of care throughout the member’s journey.
