About The Position

The Support Specialist provides essential administrative, operational, and client-service support to the Geriatric Support Services Coordinator (GSSC) team working under Senior Care Options contracts. This role ensures that care coordination activities are completed efficiently, documentation is accurate and timely, and communication flows smoothly between SCO partners, interdisciplinary care teams, consumers, caregivers, and internal ASAP departments. The Support Specialist plays a key role in helping SCO members maintain independence, access services, and receive high-quality, person-centered care.

Responsibilities

  • Assist GSSCs with scheduling assessments, home visits, and follow‑up calls with SCO members and caregivers.
  • Track due dates for assessments, reassessments, Long Term Services and Supports care plans, and other SCO deliverables to ensure compliance with contract timelines.
  • Support GSSCs in coordinating community resources and referrals.
  • Assist GSSCs with initial authorizations, reauthorizations, and expiring authorization reports for Long Term Support services.
  • Assist GSSCs with contacting providers/vendors to verify services for new members.
  • Assist GSSCs with entering Activities and Referrals and service plans into WellSky.
  • Assist with the Home Delivered Meals (HDM) "Not At Home" notifications and remind members of the HDM policy.
  • Support the GSSC with the Unable to Reach members following the SCO's workflow.
  • Serve as a point of contact for SCO care managers, providers, and internal departments regarding scheduling, documentation needs, and general inquiries.
  • Enter, update, and maintain accurate member information in ASAP and SCO databases (e.g., WellSky, MassHealth Portal, or SCO‑specific platforms) when the SCO Operations Coordinator is unavailable.
  • Organize required documents such as assessments, care plans, releases, and other member‑related materials.
  • Maintain organized electronic and physical files in accordance with HIPAA and agency policies, ensuring PHI is always secured, cabinets are locked, computers are shut down properly, and confidential materials are never left unattended.
  • Assist with mailings, faxing, scanning, and other administrative tasks related to SCO member services when the SCO Operations Coordinator is unavailable.
  • Participate in team meetings, trainings, and quality improvement initiatives.
  • Complete required in‑services and trainings at GSSSI for ongoing professional development.
