About The Position

This position is responsible for performing assessments, crisis interventions, individual behavioral health counseling, and psycho-educational/support groups to patients with mental health and/or substance use disorders as needed for inpatients, outpatients, and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Full time, outpatient therapist opportunity embedded within a primary care pediatric clinic. Our LMHC will work as a member of the pediatric primary care department to provide short term therapy to patients with clinical mental health conditions including but not exclusive of anxiety, depression, and other mood related conditions as well as ADHD and behavioral dysregulation. Conducts psychosocial assessments, provides clinical services to pediatric patients that address psychosocial, environmental, age specific and cultural issues in order to maximize emotional, social and physical well-being and the effective/efficient use of health care and community resources. LMHC will be available to Pediatricians for consultations and acute assessments during routine clinic encounters.

Requirements

Master's Degree Social Work required or Master's Degree Mental Health & Behavioral Medicine required
Licensed Independent Clinical Social Worker [LICSW - State License] - Generic - HR Only preferred
Licensed Marriage and Family Therapist [State License] - Generic - HR Only preferred
Licensed Mental Health Counselor [LMHC - State License] - Generic - HR Only preferred
Behavioral medicine/mental health or clinical social work experience 3-5 years required
Demonstrates a mastery of a range of specialized clinical diagnoses and interventions.
Conducts clinically competent complex and high-risk assessments, treatment plans, and interventions.
Demonstrates mastery in problem-solving abilities, conflict resolution, efficiency, and communication skills.
Formulates advanced psychosocial assessments and treatment plans.
Supports other staff in developing assessments and plans in complex cases.
Demonstrates advanced clinical judgment and innovation in advocating for clients.
Demonstrates advanced working knowledge of internal and external resources and refers appropriately.
Demonstrate ability to identify problems in order of urgency and initiate treatment plans towards timely solutions and appropriate time management skills.
Demonstrates the ability to articulate a social work clinical perspective in team meetings and effectively collaborates with the interdisciplinary team to comprehensively problem-solve and optimize a plan of care.

Promotes timely and concise collaboration among all disciplines and outside stakeholders to enhance patient care (including but not limited to the Ethics Committee, SDoH Committee, Office of General Counsel, Case Management department, etc.).

Nice To Haves

Experience and training with elementary aged (5-11) children as well as CBT preferred.
Bilingual skills preferred (Spanish or Arabic)

Responsibilities

Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues—Formulates biopsychosocial assessment, disposition, and treatment plans.
Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessments when violence is suspected. Plan discharge from care facility to home or other care facility.
Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Providing short-term, evidence-based psychotherapy (individual, groups) to patients in the pediatric practice in-person and virtually
Screen patients and develop comprehensive assessments and treatment plans for patients engaged in BH treatment
Providing a range of interventions to support primary care mental health and behavioral needs such as crisis intervention; care coordination; information; referrals; and safety planning.
Assessing patients presenting with new mental health and behavioral symptoms to aid in the identification of the best treatment options for them
Participating in formal and informal case reviews, seminars and / or inter- and intra-departmental peer review meetings
Working closely with the other members from the primary care practice to provide timely and appropriate care
Advocate for school related needs as applicable to behavioral health diagnoses.
Conduct Medical Certificate for pediatric primary care patients in the division of primary care to contribute to the guardian process.
Collaborate with clinical care team members, iCMP and others coordinating guardianship for pediatric primary care patients.
